Do Debt Relief programs really help?

Can anyone tell me if those businesses advertising debt relief can truly help? They state that if you owe, lets say, $20,000 in credit card debt that they have people with the expertise who could lower it by negotiating with the credit card company and you could be out of debt in 3 to 4 years. Has anyone been through this type of program and has it been successful? It's too good to be true if this can be done.
